Effective July 1, 2014, the Department of Social Services shall provide Medicaid coverage for home telemonitoring services performed by licensed home health care agencies.

Home telemonitoring services include scheduled remote monitoring of patient health data (blood pressure, heart rate, weight, oxygen level), interpretation and transmission of data to treating physicians, and follow-up care or referrals as medically necessary.

Coverage applies to Medicaid beneficiaries with serious or chronic conditions causing frequent hospitalizations, poor medication adherence, documented falls, limited support systems, access to care challenges, or who live alone for extended periods.

The Commissioner of Social Services shall establish coverage criteria based on evidence-based best practices and adopt regulations to implement the program.

All transmission, storage, and dissemination of telemonitoring data must comply with federal and state privacy, security, and confidentiality laws.
