Requires the Department of Public Health, Department of Social Services, and other state health and human services agencies to collect primary data on persons served, including race, ethnicity, gender, primary language, disability status, and income.

Data must be aggregated in a manner that does not identify individuals, with optional additional collection of residential address, socioeconomic status, and level of acculturation.

Data collection must use self-reporting methods, with exceptions allowing parents or guardians to report information on behalf of minors and guardians or conservators to report for incapacitated persons.

Takes effect January 1, 2015.
