Requires health districts with 50,000+ population or serving three or more municipalities to employ a full-time director of health, or receive a waiver from the Commissioner if a vacancy exceeds 90 days.

Establishes state funding of $1.85 per capita for qualifying health districts, contingent on Commissioner approval of public health programs and local municipal appropriations of at least $1.00 per capita.

Requires municipal health departments with 50,000+ population to employ a full-time director of health, or receive a waiver if a vacancy or acting director role exceeds 90 days, with state funding of $1.18 per capita.

Changes unexpended funds policy: funds may carry over to the next fiscal year for fiscal years ending June 30, 2014 and 2015, but must revert to the General Fund starting with fiscal year ending June 30, 2016 and thereafter.

Takes effect October 1, 2014.
