Caps copayments at a maximum of $20 for breast ultrasound screening in both individual and group health insurance policies when imaging shows dense breast tissue or patient is at increased risk for breast cancer.

Requires health insurance policies to cover comprehensive ultrasound screening and MRI of breasts based on mammogram results and American Cancer Society guidelines.

Mandates mammography reports include breast density information and a notice informing patients about supplementary screening options if dense breast tissue is detected.

Limits copayments to a maximum of $30 per visit for in-network physical therapy and occupational therapy services in both individual and group health insurance policies.

Applies to health insurance policies covering specified types of coverage and becomes effective January 1, 2015.
