Establishes a Hospital-Community Connection Program (HCCP) requiring hospitals to participate in inpatient discharge care management, intensive care management, and performance measurements to receive financial incentives under Medicaid.

Creates a supplemental HCCP payment pool funded with an annual appropriation equal to 3% of total state Medicaid hospital payments, distributed to hospitals that meet quality of care improvement indicators.

From July 1, 2014 to June 30, 2015, hospitals earn incentives for participating in discharge care management, intensive case management, or readmission reduction programs, with each program eligible for up to one-third of the payment pool.

From July 1, 2015 to June 30, 2017, hospitals can earn incentives for six programs including discharge management, readmission reduction, and reductions in COPD, heart failure, and asthma admissions, with each program eligible for up to one-sixth of the payment pool.

Hospitals receiving full annual payment rate updates must provide timely emergency department data, inpatient admission data, and HEDIS reporting; hospitals failing to provide this data receive updates reduced by 2 percentage points.
