Requires the Department of Social Services to provide Medicaid coverage for home telemonitoring services performed by licensed home health care agencies, effective July 1, 2015.

Defines home telemonitoring service as remote monitoring of patient health data (blood pressure, heart rate, weight, oxygen level) with interpretation and dissemination to treating physicians and follow-up care as medically necessary.

Covers Medicaid beneficiaries with serious or chronic conditions, poor medication adherence, recent falls, limited support systems, access to care challenges, or extended periods living alone.

Requires the commissioner to establish coverage criteria based on evidence-based best practices and adopt implementing regulations.

Mandates that all data transmission, storage, and dissemination comply with federal and state privacy, security, and confidentiality laws.
