Creates new requirements for health care provider advertisements to clearly identify the provider's type of license, registration or certificate and prohibit deceptive or misleading information, effective October 1, 2015.

Requires health care facilities and offices to maintain and provide upon request an up-to-date list of all providers including their names, credentials, and detailed education, training and experience information.

Mandates that health care providers who provide direct patient care wear visible photographic identification badges displaying the facility name, provider name, and license or employment title during working hours, with facilities developing badge policies in consultation with the Department of Public Health.

Designates violations of the advertising and identification requirements as unfair trade practices under chapter 735a of the general statutes.

Applies to all health care providers as defined in section 20-7b and health care facilities including hospitals, nursing homes, home health agencies, surgical facilities, and educational institution infirmaries.
