Repeals and replaces Section 38a-472f of Connecticut general statutes effective October 1, 2015.

Requires insurers, health care centers, managed care organizations, and preferred provider networks to maintain provider networks consistent with National Committee for Quality Assurance (NCQA) network adequacy requirements or URAC's provider network access and availability standards.

Mandates that provider networks be adequate to meet enrollees' comprehensive health care needs and provide appropriate choice of health care providers for covered services.

Applies to entities that deliver, issue, renew, amend, or continue individual or group health insurance policies and medical benefits plans.
