Establishes the Connecticut Uniform Recognition of Substitute Decision-Making Documents Act, effective October 1, 2015, creating uniform rules for recognizing documents that authorize decision-makers to act on behalf of individuals regarding property, health care, and personal care.

Substitute decision-making documents executed outside Connecticut are valid in the state if they complied with the law of the jurisdiction where executed or indicated in the document.

Persons accepting substitute decision-making documents in good faith and without actual knowledge of invalidity may assume the document is genuine, valid, and still in effect without further inquiry.

Persons asked to accept a substitute decision-making document must do so within a reasonable time if it meets validity requirements, except in specific circumstances including actual knowledge of termination, good faith belief of invalidity, or reports of potential abuse or exploitation to relevant authorities.

Persons who wrongfully refuse to accept a valid substitute decision-making document are subject to court-ordered acceptance and liable for reasonable attorney's fees and costs in enforcement actions.
