Requires children to be immunized against diphtheria, pertussis, tetanus, poliomyelitis, measles, mumps, rubella, hemophilus influenzae type B, and other vaccines per state schedule before enrolling in public or nonpublic schools, and requires a second measles immunization before seventh grade.

Establishes religious exemptions from immunization requirements but mandates that religious objection statements be notarized or officially acknowledged by a judge, magistrate, clerk, town clerk, notary public, justice of the peace, or attorney.

Requires parents or guardians of school-age children with religious exemptions to annually resubmit an officially acknowledged statement to remain enrolled in school programs.

Applies the same notarized religious exemption requirements to child day care centers, group day care homes, and family day care homes under regulations adopted by the Commissioner of Early Childhood.

Requires child day care providers to obtain annual notarized religious exemption statements from parents or guardians to continue providing services to exempt children, effective July 1, 2015.
