Allows the Connecticut Department of Public Health Commissioner to issue new birth certificates reflecting gender changes upon receipt of a written request, notarized affidavit from a licensed physician, advanced practice registered nurse, or psychologist confirming the applicant has undergone surgical, hormonal, or other clinically appropriate treatment for gender transition, and proof of legal name change if applicable (effective October 1, 2015)

Grants probate courts in Connecticut jurisdiction to issue decrees allowing residents born in other states or foreign jurisdictions to amend their birth certificates to reflect gender changes using the same documentation requirements as Connecticut-issued certificates

Eliminates the previous requirement that applicants obtain separate affidavits from physicians regarding physical changes and from psychologists/psychiatrists regarding social and psychological changes, consolidating into one notarized professional affidavit

Requires probate court decrees to be transmitted to the registration authority of the person's place of birth for out-of-state birth certificates

Preserves the Commissioner of Public Health's authority to amend birth certificates under the revised procedures
