Repeals and replaces Section 20-7c of general statutes to establish requirements for providers to supply patients with complete medical information, test results, and health records upon request, effective October 1, 2015.

Limits provider charges to 65 cents per page plus a $20 research and handling fee (with no fee for copies to patients), postage costs, x-ray materials costs, and up to $10 for certification; prohibits charges when records support Social Security Act claims or appeals.

Requires providers to furnish requested health records within 30 days of request and prohibits providers from refusing to return original records or copies that patients brought from other providers.

Allows providers to withhold information only if reasonably determined to be detrimental to patient's physical or mental health or likely to cause harm, with patients able to petition superior court within 30 days to challenge such withholding.

Amends Section 19a-490b governing institutional health records to establish similar fees and timeframes (30 days), require institutions to provide access to retained tissue slides and blocks, and mandate certified documentation of record storage procedures when institutions cease operations.
