Establishes a grant program administered by the Commissioner of Rehabilitation Services to provide assistance for home modifications and assistive technology devices for eligible persons.

Defines eligible persons as those age 65 or older, or any age with physical disabilities, with annual income not exceeding 400% of the federal poverty level, and with a written statement from a licensed health care provider documenting need for home modifications or assistive technology.

Authorizes the Commissioner to assign staff to administer the grant program for purposes of allowing eligible persons to age in place in a home setting of their choice.

Appropriates $5 million from the General Fund to the Department of Rehabilitation Services for fiscal year ending June 30, 2016, to establish and administer the grant program.

Takes effect July 1, 2015, and amends section 17b-607 of the general statutes.
