Adds a new patient right requiring nursing homes, residential care homes, and chronic disease hospitals to provide patients with copies of any Medicare or Medicaid applications completed on their behalf, or allow patients to designate a representative to receive the copy.

Effective July 1, 2015.

Restructures the existing patients' bill of rights by renumbering sections (27) through (29) to accommodate the new Medicare/Medicaid application requirement as section (26).
