Requires electors who complete an affidavit form instead of presenting photo identification to cast a provisional ballot rather than a regular ballot, effective July 1, 2016.

Mandates the Secretary of the State establish procedures to audit all affidavit forms signed by voters within 30 days after each primary, election, or referendum for accuracy.

Requires the Secretary of the State to submit audit findings to the joint standing committee on elections within 30 days of completing the audit.

Clarifies that voters without preprinted identification who complete the affidavit form under subsection (a) of section 9-261 receive a provisional ballot that is counted according to provisional ballot procedures under sections 9-232i to 9-232o.
