Commissioner of Social Services shall seek federal approval for a Section 1115 Medicaid waiver within 60 days for a five-year community-based diabetes prevention education program, with application deadline set for 60 days after the effective date.

The diabetes prevention program may be contracted to nonprofit community-based providers and shall include information on dietary changes, weight management techniques, and importance of regular health checkups.

Commissioner of Social Services shall coordinate the federal waiver program with the state's Diabetes Prevention and Control Program in consultation with the Commissioner of Public Health.

Commissioner shall submit annually to relevant legislative committees a report on program enrollment numbers and health outcomes measured by weight and blood glucose management.

Commissioner of Social Services shall seek federal approval for a Medicaid state plan amendment by September 1, 2016 to provide reimbursement for diabetes self-management training delivered under physician-approved care plans to Medicaid recipients.
