Repeals and replaces Section 19a-6q of the general statutes to require the Commissioner of Public Health to develop and implement a chronic disease reduction plan consistent with Healthy Connecticut 2020 and the State Innovation Model Initiative.

Commissioner must submit biennial reports to the joint standing committee on public health detailing chronic disease prevalence, funding sources, care coordination efforts, and prevention recommendations.

Requires Commissioner of Public Health, in collaboration with the Commissioner of Social Services and State Comptroller, to identify existing state diabetes programs, assess their effectiveness, and evaluate the financial impact of diabetes on the state by December 1, 2016.

Commissioner of Public Health must develop a diabetes action plan setting forth steps to reduce diabetes impact, including expected outcomes and benchmarks, with annual implementation reports due starting January 1, 2017.
