Authorizes the Commissioner of Social Services to implement an acuity-based methodology for Medicaid reimbursement of nursing home services, effective July 1, 2016.

Requires the commissioner to review the skilled nursing facility prospective payment system developed by the Centers for Medicare and Medicaid Services and other national methodologies, and to seek federal approval through a Medicaid state plan amendment if necessary.

Establishes potential components of the acuity-based methodology including case mix classification systems, resident assessment instruments, rate adjustments for special needs residents (dementia, ventilator-dependent, developmental disabilities, behavioral health, bariatric), county cost differentials, and pay-for-performance adjustments.

Mandates a phase-in period of at least four years with protections limiting decreases in Medicaid per diem rates for nursing facilities during implementation.

Requires the Commissioner of Social Services to report annually to the joint standing committee on human services regarding implementation and the rate change impact on each facility.
