Authorizes the Department of Social Services to provide Medicaid coverage for home telemonitoring services through the Money Follows the Person demonstration project, effective July 1, 2016.

Defines home telemonitoring service as remote monitoring of patient health data (blood pressure, heart rate, weight, oxygen level) by a licensed home health care agency, with data interpretation and transmission to treating physicians.

Establishes eligibility criteria for coverage including patients with serious or chronic conditions prone to hospitalization, poor medication adherence, documented falls history, limited support systems, access to care challenges, or prolonged periods living alone.

Requires the department to establish coverage criteria based on evidence-based best practices and ensure clinical information is shared with the patient's treating physician while complying with federal and state privacy and security laws.

Mandates the department study the impact on health outcomes, costs, beneficiary independence, and quality of life, with findings reported to the joint standing committee on human services by January 1, 2018.
