Establishes an all-payer claims database program overseen by the exchange to collect and store health care claims data from insurers, health care centers, third-party administrators, pharmacy benefits managers, and other reporting entities.

Requires reporting entities to submit health care information in a form and manner prescribed by the exchange, with civil penalties up to $1,000 per day for non-compliance.

Authorizes the exchange to provide database access to state agencies, insurers, employers, health care providers, consumers, and researchers for reviewing health care utilization, costs, and quality, with the exchange able to charge access fees.

Adds requirement for the exchange to develop and implement a form allowing individuals to exclude their health care service data from all-payer claims database reporting.

Creates the All-Payer Claims Database Advisory Group to develop a state-wide multipayer data initiative to enhance efficiency and outcomes in both public and private health care sectors; effective October 1, 2016.
