Eliminates copayments and deductibles for mammograms and comprehensive breast ultrasound examinations under individual and group health insurance policies, effective January 1, 2017.

Requires coverage of baseline mammograms for women ages 35-39 and annual mammograms for women age 40 and older.

Mandates coverage of comprehensive breast ultrasound examinations for women with dense breast tissue or increased breast cancer risk factors, as determined by a physician or advanced practice registered nurse.

Prohibits annual or lifetime limits on the dollar value or number of visits for covered mammograms and ultrasound examinations.

Requires mammography reports to include breast density information and a notice about supplementary screening options for patients with dense breast tissue.
