Requires nursing home license applicants to submit applications in the form and manner prescribed by the Commissioner of Public Health, effective October 1, 2016.

Mandates the commissioner include a notice on the first page of applications stating that nursing home licensees, owners, officers, and administrators may face civil, criminal, and administrative liability for abuse or neglect of residents by employees.

Requires applications to disclose whether applicants have received three or more civil penalties in the preceding two years, Medicare/Medicaid sanctions exceeding $20,000, or had their Medicare/Medicaid provider agreements terminated or not renewed.

Prohibits the commissioner from approving applications for five years if the applicant has disclosed civil penalties, sanctions, terminations, or nonrenewals, unless good cause is demonstrated.
