Department of Social Services shall provide Medicaid coverage for home telemonitoring services provided by home health care agencies, effective July 1, 2016.

Coverage applies to beneficiaries with serious or chronic medical conditions, documented poor medication adherence, documented falls history in prior six months, limited support systems, access to care challenges, or extended periods living alone.

Home telemonitoring service requires scheduled remote monitoring of health data (blood pressure, heart rate, weight, oxygen level), interpretation by a home health care agency, data dissemination to treating physician, and follow-up care or referrals as medically necessary.

Department shall establish coverage criteria based on evidence-based best practices and ensure clinical information is shared with the patient's treating physician.

Commissioner may seek federal Medicaid waiver or state plan amendment if necessary; data transmission and storage must comply with federal and state privacy, security, and confidentiality laws.
