Allows nursing home residents to designate an unpaid caregiver (relative, friend, neighbor, or partner) to assist with post-discharge care at their home.

Requires nursing home facilities to record the caregiver designation in the discharge plan and make multiple reasonable attempts to notify the designated caregiver of the resident's discharge.

Mandates nursing home facilities provide pre-discharge training to designated caregivers on all post-discharge assistance tasks, including written or in-person demonstrations, question-and-answer sessions, and culturally competent language access services.

Protects nursing home facilities and their staff from liability for services rendered or not rendered by the designated caregiver after discharge.

Designates caregivers cannot be reimbursed by government or commercial payers for post-discharge assistance, and the act does not affect insurance coverage obligations or advance directives.
