Commission on Human Rights and Opportunities shall create and maintain a one-page disclosure form written in plain language containing information on housing discrimination and federal and state fair housing laws, making it available on their website.

Persons offering residential property with two or more units for sale, exchange, or lease with option to buy must require prospective purchasers to sign the disclosure form at the time of entering into a purchase agreement, option, or lease with purchase option.

Property sellers must provide a copy of the signed disclosure form to the prospective purchaser at the time of closing.

Failure to comply with disclosure requirements does not void an otherwise valid purchase agreement, option, or lease containing a purchase option.
