Requires youth athletic activity operators to provide written or electronic information and consent forms regarding concussions to athletes and parents/guardians by January 1, 2018, and annually thereafter, consistent with CDC guidance.

Mandates that coaches of youth athletic activities complete concussion training through the CDC or a substantially equivalent course prior to coaching, effective January 1, 2018.

Requires coaches to immediately remove youth athletes exhibiting signs, symptoms, or diagnosed concussions from participation and notify parents/guardians within 24 hours.

Prohibits youth athletes removed for concussions from participating in physical activities until they show no symptoms at rest or with exertion and receive written clearance from a licensed health care professional trained in concussion evaluation.

Expands the definition of "licensed health care professional" to include chiropractors licensed under chapter 372, in addition to physicians, physician assistants, advanced practice registered nurses, and athletic trainers.
