Requires parents or guardians claiming religious exemption from school immunization requirements to complete an immunization education program administered by the Department of Public Health before the exemption is granted.

Mandates the Department of Public Health establish an audio-visual immunization education program covering vaccine safety, efficacy, and herd immunity principles, made publicly available on the department's website.

Requires religious exemption statements to be acknowledged by authorized officials including judges, court clerks, town clerks, notaries public, justices of the peace, attorneys, or school nurses.

Effective October 1, 2017, applies to Section 10-204a of the general statutes governing school immunization requirements for diphtheria, pertussis, tetanus, poliomyelitis, measles, mumps, rubella, and hemophilus influenzae type B.
