Requires students to receive a booster dose of meningococcal conjugate vaccine before entering twelfth grade, in accordance with CDC Advisory Committee on Immunization Practices recommendations.

Adds meningococcal vaccine to existing school immunization requirements for diphtheria, pertussis, tetanus, poliomyelitis, measles, mumps, rubella, and hemophilus influenzae type B.

Extends religious exemption documentation requirements to twelfth grade entry, requiring parents or guardians of exempt students to present an acknowledged statement of religious objection before both seventh and twelfth grade enrollment.

Maintains existing exemptions for medical contraindications, religious beliefs, confirmed disease history, and age-related exemptions for pertussis and hemophilus influenzae type B.

Takes effect January 1, 2018.
