Department of Public Health shall establish a pilot program in no more than three municipalities by January 1, 2018, allowing emergency medical services personnel to provide community-based health care services.

Pilot program services include transporting patients to non-emergency department facilities, providing post-discharge home visits, conducting home visits for frequent emergency department users with chronic diseases, and delivering primary care in medically underserved areas.

Pilot program terminates on July 1, 2019.

Commissioner of Public Health must report to the joint standing committee on public health by January 1, 2020, assessing the program's effectiveness and providing legislative recommendations for statewide implementation.
