Removes the October 1, 1983 finalization date restriction, allowing all adopted persons age 18 or older to request uncertified copies of their original birth certificates regardless of when their adoption was finalized

Permits adult children and grandchildren of adopted persons to request original birth certificates on behalf of the adopted person

Requires the Commissioner of Public Health to submit annual reports to the joint standing committee on public health from January 1, 2018 through January 1, 2023 regarding the number of original birth certificates issued

Requires the Commissioner of Children and Families to submit annual reports from January 1, 2018 through January 1, 2023 on contact preference forms and medical health history forms filed by birth parents

Allows adopted persons to petition a court to compel the child-placing agency to provide identifying information if the Department of Public Health cannot locate the original birth certificate

All provisions effective July 1, 2017
