Amends living will and health care instruction document forms to include pregnancy-specific options for pregnant patients exercising advance directives.

Pregnant patients may choose one of three options: (1) accept life support if doctor believes fetus can reach live birth, (2) have the document apply without modifications, or (3) specify alternative instructions.

Applies pregnancy provisions to both basic living will documents (Section 19a-575) and comprehensive health care instruction documents that include health care representative appointments and anatomical gifts (Section 19a-575a).

Requires at least two witnesses for document execution and permits health care providers to rely on duly executed documents unless they receive actual notice of revocation.

Mandates that comfort care and pain alleviation be provided in all cases, regardless of other advance directive provisions.
