Expands director of health qualifications effective July 1, 2018 to include physicians (MD/DO), advanced practice nurses (nurse practitioners, clinical nurse specialists, or nurse anesthetists), or persons with graduate degrees in public health from accredited institutions.

Requires Commissioner of Public Health approval for appointment of directors of health in municipal and district health departments, and grants commissioner authority to appoint acting directors or directors if local entities fail to do so within 60 days.

Changes sanitarian requirements from "certified" to "licensed" under chapter 395 and replaces references to "Public Health Code" with "regulations of Connecticut state agencies" throughout relevant statutes.

Modifies district health department withdrawal requirements from 24 months to 36 months minimum membership duration, and requires withdrawing municipalities to hire full-time directors of health and provide basic health programs.

Replaces four-year director of health terms with written agreements between boards and directors, removes three-year maximum limits, and requires annual reports to Department of Public Health on provision of basic health programs.
