Requires individual and group health insurance policies to cover essential health benefits including ambulatory services, emergency care, hospitalization, maternity care, mental health services, prescription drugs, rehabilitation services, lab services, preventive care, and pediatric services, effective January 1, 2019.

Mandates coverage of comprehensive women's health benefits including domestic violence screening, tobacco cessation counseling, well-woman visits, breast cancer screening and genetic testing, sexually transmitted infection screening, pregnancy-related screening and care, breastfeeding support and supplies, and osteoporosis screening without copayments or deductibles.

Requires coverage of immunizations recommended by the American Academy of Pediatrics, American Academy of Family Physicians, American College of Obstetricians and Gynecologists, and the CDC's Advisory Committee on Immunization Practices without cost-sharing.

Expands contraceptive coverage to include all FDA-approved contraceptive drugs and devices, over-the-counter contraceptives, female sterilization methods, follow-up care, and counseling with no out-of-pocket costs, with exceptions for religious employers and individuals with religious or moral objections.

Provides preventive care and screenings for individuals age 21 and younger according to the American Academy of Pediatrics' "Bright Futures" guidelines without cost-sharing, and eliminates annual and lifetime benefit limits on essential health benefits.
