Increases advance notice requirement from 60 to 90 days when health carriers remove participating providers from networks or providers leave networks, effective July 1, 2018

Requires health carriers to notify affected patients within 30 days of a provider removal or departure, regardless of whether the removal was for cause

Mandates health carriers and hospital participating providers continue abiding by existing contract terms, including reimbursement rates, for 60 days following termination or non-renewal of contracts

Establishes continuity of care procedures for patients in active treatment, including medically necessary ongoing care, pregnancy-related care, or complex conditions, allowing patients to continue with departing providers for up to 90 days unless longer periods are medically necessary

Requires health carriers to provide patients with lists of alternative in-network providers in their geographic area when a treating provider leaves the network
