Allows adopted persons age 18 and older to request uncertified copies of original birth certificates from the Department of Public Health without requiring a court order, regardless of when the adoption was finalized.

Extends access to original birth certificates to adult children and grandchildren of adopted persons under the same conditions.

Removes the October 1, 1983 finalization date restriction that previously limited access only to adoptions finalized after that date.

Requires original birth certificates to include notice that birth parent contact preferences and medical health history forms may be on file with the Department of Children and Families.

Establishes reporting requirements for the Commissioners of Public Health and Children and Families to track issued original birth certificates and filed contact preference forms through January 1, 2024.
