Defines "spontaneous fetal demise" as the death of a fetus occurring before 20 weeks of gestation.

Grants mothers the right to arrange burial or cremation of a fetus that experienced spontaneous fetal demise, with disposition subject to the same laws and regulations as fetuses dying at or after 20 weeks of gestation.

Requires hospitals with custody of a fetus following spontaneous fetal demise to notify the mother in writing of her burial and cremation rights.

Allows mothers 24 hours from hospital notification to elect in writing whether to arrange burial or cremation of the fetus.

Directs the Department of Public Health to develop notification and election forms for hospitals to provide to mothers, effective July 1, 2018.
