Requires individual and group health insurance policies to cover prosthetic devices (artificial limbs) at least equivalent to Medicare coverage, effective January 1, 2019.

Includes coverage for microprocessor-equipped prosthetic devices when determined medically necessary by a health care provider, but excludes devices designed exclusively for athletic purposes.

Mandates coverage for medically necessary repair or replacement of prosthetic devices unless the need results from misuse or loss.

Prohibits coinsurance, copayment, deductible, or out-of-pocket expenses for prosthetic devices that are more restrictive than those imposed on substantially all other policy benefits, with an exception for high deductible health plans.

Permits insurers to require prior authorization for prosthetic devices only if applied in the same manner and extent as for other covered benefits.
