Department of Public Health shall develop and make available a standardized form by October 1, 2019 for parents or guardians to present religious objections to required immunizations under section 10-204a of the general statutes.

The form must be acknowledged by authorized officials including judges, court clerks, town clerks, notaries public, justices of the peace, attorneys, or school nurses, with the form to be published on the department's website.

Form must include a notice stating that any person authorized to acknowledge the statement has discretion to acknowledge or refuse to acknowledge it.

Department of Education shall send written notification by September 1, 2019 to all local and regional boards of education and the Association of School Nurses of Connecticut informing school nurses of their discretion to acknowledge or refuse such statements.

Local and regional boards of education must distribute the notification to all school administrators as defined in section 10-144e of the general statutes.
