Renames the "Department on Aging" and merges it with rehabilitation services to create the "Department of Aging and Disability Services," effective October 1, 2019.

Updates all references throughout Connecticut statutes to replace "Commissioner on Aging" with "Commissioner of Aging and Disability Services" and "Department on Aging" with "Department of Aging and Disability Services."

Designates the new Department of Aging and Disability Services as the State Unit on Aging and authorizes it to administer programs including elderly nutrition, fall prevention, CHOICES health insurance assistance, Aging and Disability Resource Center, and Alzheimer's respite care programs.

Requires the Commissioner of Social Services to increase meal reimbursement rates for meals-on-wheels providers annually by at least the cost-of-living adjustment as measured by consumer price index, effective July 1, 2020.

Transfers all functions, powers, duties, and personnel from the former Department on Aging to the new consolidated Department of Aging and Disability Services, with all existing orders and regulations continuing in effect until amended or repealed.
