Grants the Department of Public Health remote access to electronic medical records at hospitals for cases involving reportable diseases, emergency illnesses, or health conditions listed by the Commissioner of Public Health, with access methods approved by the Commissioner.

Requires health care providers to report cases of reportable diseases to local directors of health and the Department of Public Health within 12 hours of recognition, in writing, by telephone, or in an electronic format approved by the Commissioner.

Requires clinical laboratories reporting an average of more than 30 findings per month to submit reports electronically in Commissioner-approved format, while those reporting fewer than 30 findings per month may submit reports in writing, by telephone, or electronically.

Maintains strict confidentiality protections for all personal information obtained from disease prevention investigations, with violators subject to fines of up to $500.

Expands the Department of Public Health's access to hospital records to perform case finding and quality improvement audits to ensure completeness of reporting and data accuracy, effective October 1, 2019.
