Makes permanent the moratorium on Office of Higher Education approval for new programs and program modifications at independent institutions of higher education, removing the previous July 1, 2020 expiration date.

Allows up to twelve new programs per academic year and any program modifications to proceed without Office approval, provided institutions meet three conditions: maintain Title IV, Part B financial aid eligibility, have a financial responsibility score of at least 1.5 from the U.S. Department of Education, and have been regionally accredited for ten or more years.

Requires institutions exempt from program approval to submit an application for any new programs exceeding twelve per academic year.

Mandates institutions file a program actions form with the Office of Higher Education before students enroll in any new or modified program.

Requires annual reporting by July 1 listing all new and discontinued programs, the institution's current program approval process, governing board actions on program approval, and the institution's most recent financial responsibility composite score.
