Removes the October 1, 1983 adoption finalization date restriction, allowing all adult adopted persons (age 18+) to request uncertified copies of original birth certificates from the town registrar of vital statistics within 30 days of written request.

Extends access rights to adult children and grandchildren of adopted persons to request original birth certificates.

Requires original birth certificates to include notation that the document has been superseded and a notice about birth parent contact preference information and medical health history forms filed with the Department of Children and Families.

Extends reporting requirements for the Commissioner of Children and Families to January 1, 2025 (previously January 1, 2021) regarding contact preference forms and medical health history forms filed annually.

Eliminates the need for court petition process for adopted persons whose adoption was finalized on or after October 1, 1983, while maintaining court petition option for pre-1983 adoptions.
