Amends section 19a-323(b) of Connecticut general statutes to modify cremation permit fee procedures, effective July 1, 2020.

Sets cremation permit fee at $5 and written notice fee at $1, except no fees shall be charged for cremation of deceased persons under age 18.

Eliminates cremation certificate fees for stillborn fetuses and deceased persons under age 18; Office of the Chief Medical Examiner retains authority to waive fees upon request.

Maintains existing procedures for registrars to authorize cremation permits and forward cremation certificates to appropriate town registrars, including after-hours subregistrar authority.

Requires Department of Public Health to provide cremation permit forms distinct from burial permit forms, including space to record information about disposition of cremated remains.
