Department of Public Health shall distribute an immunization survey to child care centers, group child care homes, and family child care homes by January 15, 2021, and annually thereafter, with questions about enrolled children's immunization status according to the state's active immunization schedule.

Survey shall not request any personally identifying information about individual children.

Child care facility licensees must complete and return the immunization survey by February 15, 2021, and annually thereafter in a form prescribed by the Department of Public Health.

Department of Public Health shall review survey responses by June 1, 2021, and annually thereafter to determine whether herd immunity exists for each disease at each facility and compile a published list of findings on the department's website.

Child care facility licensees may request in writing to be omitted from the published list.
