Establishes a state-funded pilot program through the Commissioner of Social Services to serve no more than 100 persons with disabilities aged 18-64 who are inappropriately institutionalized or at risk of institutionalization.

Limits eligibility to individuals whose assets do not exceed the asset limits of the state-funded home care program for the elderly established under section 17b-342.

Excludes persons who are eligible for medical assistance under section 17b-261 or Medicaid waivers pursuant to 42 USC 1396(c) from receiving services under this program.

Allows eligible participants to receive the same services provided under the state-funded home care program for the elderly, plus disability-related services at the Commissioner's discretion to help avoid institutionalization.

Takes effect upon passage and amends section 17b-617(a) of the general statutes.
