Extends guardianship duration for children in Department of Children and Families custody from age 18 to age 21 if the child is in full-time attendance at a secondary school, technical education and career school, college, or state-accredited job training program.

Requires the Commissioner to give priority to placing children with grandparents if the grandparents agree and meet the commissioner's standards, and if grandparent placement is not possible, to select a placement of like religious faith to the child's parents.

Mandates written notification to the child's attorney and guardian ad litem within ten business days prior to placement changes in non-emergency situations, or within two business days following emergency placement changes.

Requires at least ten days written notice to the child and child's attorney before transferring a child to a second or subsequent placement, unless an emergency necessitates immediate transfer.

Requires courts within sixty days of commitment to determine whether the Department made reasonable efforts to keep the child with parents or guardians prior to the commitment order.
