Defines "complex care navigator" as a community health worker with experience in health care coordination for persons with complex medical needs, including cancer diagnoses, who meets qualification requirements established by the Office of Health Strategy.

Requires the Commissioner of Social Services to establish a pilot program by October 1, 2020, incorporating complex care navigators into the Department of Social Services' Person-Centered Medical Home Plus program, in consultation with the Commissioner of Public Health.

Mandates the Commissioner of Social Services to report annually starting January 1, 2022, to joint standing committees on the pilot program's effectiveness in increasing health outcomes for participants.

Establishes the Office of Health Strategy's responsibility to study the feasibility of creating a certification program for community health workers, including training requirements, administration methods, and training program curricula recognition.

Requires the Office of Health Strategy to establish qualification requirements for community health workers seeking to participate as complex care navigators in the pilot program by October 1, 2020.
