Prohibits nursing homes and residential care homes from involuntarily discharging or transferring residents to homeless shelters or temporary/unstable housing, defined as hotel/motel stays under 30 days, housing without legal occupancy rights, or situations where health needs cannot be met or no caregiver is available.

Requires facilities to provide residents with 30-60 days written notice before transfer or discharge (except emergencies), including reasons, effective date, appeal rights, and contact information for ombudsman and advocacy services.

Establishes appeal process allowing residents to request hearings before the Commissioner of Social Services within 60 days of notice; involuntary transfers are stayed pending commissioner's decision.

Requires development of discharge plans considering resident preferences, proximity to relatives, and available care and services; physician or advanced practice registered nurse must document basis for transfer/discharge.

Applies to facilities participating in Medicaid or Medicare programs, chronic and convalescent nursing homes, rest homes with nursing supervision, and residential care homes; takes effect upon passage.
