Prohibits hospitals and health systems from collecting facility fees for outpatient services using CPT E/M or A/M codes at off-campus hospital-based facilities, and caps facility fees for uninsured patients at off-campus locations to no more than the Medicare rate

Requires written notices to patients about facility fees to be provided in at least the top 15 languages spoken in Connecticut starting October 1, 2021, and mandates hospitals submit sample billing statements annually to the Office of Health Strategy

Strengthens patient notification requirements when healthcare facilities are acquired by hospitals, requiring written notice within 30 days to all patients served in the preceding three years, with facility fee collection prohibited until 30 days after notification

Expands annual facility fee reporting requirements to include data disaggregated by payer mix (Medicare, Medicaid, private insurance, self-pay), gross and net revenue figures, and starting July 1, 2023, data on patient requests for facility fee reductions

Billing statements must clearly identify facility fees, compare them to Medicare rates, inform patients their costs may have been lower at non-hospital facilities, and notify patients of their right to request fee reductions with a contact phone number
