Expands the definition of "amendment" to vital records to include creating replacement marriage certificates for gender changes, in addition to existing provisions for birth certificates

Authorizes the Commissioner of Public Health to amend marriage certificates to reflect gender changes, with the original certificate sealed and kept confidential

Requires applicants seeking a marriage certificate gender change to submit: a written request signed under penalty of law, notarized spousal consent, a notarized affidavit from a licensed physician/APRN/psychologist confirming gender transition treatment, and proof of any legal name change

Original marriage certificates amended for gender change will be sealed and replaced with new certificates that do not reveal the original information

Effective date: October 1, 2021
